How to add a form within a form

This video is going to show you how to add a form within a form. You may want an action in a form to trigger filling out another form. This is how you do it without having the person disrupt their workflow.

How to add a form within a form

  1. Click Forms Library in the left-hand navigation.

  2. Create a new form or find the form you would like to edit.

    • In this example, we will edit an Incident Report with Investigation form. We want the Injury Report form to display if Injury is selected when the form is being filled out.

  3. To do so, highlight the section, click add form element, select widget, and sub form.

  4. Click on the sub form, change the label, then locate the form you want to display.

  5. To make this only display if the injury checkbox is selected, click conditions, enter the condition parameter, equals true, and set the action to visible.

  6. Once all looks good, click save and publish.

    • Now when we go to fill out this form, we’ll see that if we click the injury checkbox, the Injury Report form displays.
