How to add a corrective action to a form

This video is going to show you how to add a corrective action to a form. There are two ways to do this. Let’s look at both options in our hazard report example.

The first is with an Inspection question.

1. By toggling on the allow action creation, you are enabling a corrective action.

2. Select the corrective action template you’d like to display in the allowed corrective actions drop down.

   • If this is left blank, all active corrective action templates will display.

3. Let’s select corrective action, click save and publish. And let’s see what it looks like in action.

The second way to add a corrective action is to add a subform. Let’s go back to the builder and remove the allow creation toggle from our inspection question.

1. Let's highlight this question, add form element, select widgets, and add a sub form.

2. Highlight the subform, Change the label name, select corrective action in the sub form or corrective action drop down, and select the correct template.

3. Next, we will add a condition to only make this corrective action display if the question has this hazard been appropriately controlled is answered no.

4. To do so, find the question in the condition parameter, equal, no, action, visible.

5. Once all looks good, click save and publish. Now if we fill this form and answer no to this question, you will see the corrective action displays.
